Case Management
Case Management connects you to services and resources throughout the community.
Case management is a collaborative process that helps people achieve their goals by coordinating and integrating services and support. It can be used in various settings, including healthcare, mental health care, and social services.
Our case manager will visit you at home, or you can come to the Senior Activity Center. At your first appointment, the case manager will assess your needs and connect you to services and resources throughout the Los Alamos community and within the state.
They will determine your eligibility for home-delivered meals, Adult Day Out services, Transportation, and social assistance programs. Case Management also provides follow-up and assistance with your changing needs, such as safety, benefits, access to medical equipment, and more.
The Case Manager is available to individuals who live in Los Alamos County and are members of the Senior Activity Centers.
